Care Management

WebVMC provides the healthcare industry with a proven solution for improving clinical outcomes while decreasing today’s staggering healthcare expenses. Through our care management plans, WebVMC is able to assist family members and caregivers in finding the right solution to fit each individual’s needs.  We work hands-on with our clients to determine which patients would benefit the most from our solution based upon historical disease state patients, case mix, re-hospitalizations and emergent care.

WebVMC’s solution offers the patient a customized care management plan which will effectively manage their disease state in conjunction with the case manager’s care plan. The RemoteNurse™ supports a variety of medical devices which assist clients in monitoring disease states such as CHF, COPD, diabetes, asthma, hypertension, and obesity.

An in-house client study by Piedmont Hospital in Atlanta, Georgia supports the cost effectiveness of the WebVMC RemoteNurse solution.  Piedmont has documented a reduction in hospitalizations and a decrease in ER visits for their CHF population since partnering with WebVMC in 2004.  See Piedmont Hospital - Fuqua Heart Failure Resource Center release

Disease States and Treatment

 

CHF – Congestive Heart Failure

 

Heart failure (sometimes known as congestive heart failure [CHF]) is a serious condition in which the heart is not pumping efficiently. It is a chronic condition that is the result of other cardiac conditions. In the late stages of heart failure, the heart is unable to meet the body’s demand for oxygen. In addition, it may cause congestion in the lungs or other problems throughout the body. As a result of the lack of oxygen-rich blood flowing to the body, the heart tries to work harder, which only makes the problem worse.

 
According to the American Heart Association, nearly 5 million Americans are living with heart failure, and 550,000 new cases are diagnosed each year.  In fact, CHF is one of the most common reasons that people that are age 65 and over go into the hospital. Pulmonary hypertension is characterized by high blood pressure (hypertension) in the blood vessels that supply oxygen-poor blood to the lungs. There are two forms of pulmonary hypertension: primary and secondary. Primary pulmonary hypertension (PPH) has no known cause. Secondary pulmonary hypertension (SPH) is caused by an underlying heart condition, lung disease or pulmonary embolism (caused by a blood clot that has become lodged in one or more of the pulmonary blood vessels). Secondary pulmonary hypertension has also been associated with the use of certain diet drugs.

COPD - Chronic Obstructive Pulmonary Disease

 

Chronic obstructive pulmonary disease (COPD) is a chronic, progressive disease of the lungs that gradually reduces airflow. It is characterized by phlegm coughing, wheezing and shortness of breath. As the disease progresses, quality of life may be severely compromised.
 

COPD is one of the five most lethal diseases in the world. Both its mortality and its frequency are increasing. The disease is closely associated with cigarette smoking. Studies have shown that smoking is associated with up to a 20-fold increase in the risk of death from COPD. Smoking can lead to the two most common forms of this disease, emphysema and chronic bronchitis.

Asthma

 

According to the asthma and Allergy Foundation of America, more than 20 million Americans suffer from asthma.  Furthermore, every day in America 5,000 people visit the emergency room and 1,000 people are admitted to the hospital due to this disease. When an individual suffers from asthma, two main things are happening in their lungs:

 

• constriction (the tightening of the muscles around the airways); and

• inflammation (the swelling and irritation of the airways).

 

Constriction and inflammation narrow your airways, which can cause symptoms such as wheezing, coughing, chest tightness, and shortness of breath. There is increasing evidence that, if left untreated, asthma may cause a long-term decline in lung function.

Diabetes

 

Type 2 diabetes is the most common type of diabetes. It occurs when glucose builds up in the blood due to the body's inability to use insulin effectively. Insulin is a hormone that is essential to transport glucose, a sugar that is the body's primary fuel, into the cells.

 

Type 2 diabetes was once called adult onset diabetes because it is usually diagnosed in adulthood. Excess fat, inactivity and poor eating habits contribute to type 2 diabetes. Sometimes the disease can be prevented or controlled through diet and exercise, but some patients need insulin or other medications.

Hypertension

Obesity

 

Obesity is a condition in which a person has a body mass index of 30 or greater. It is the second leading cause of preventable death, contributing to serious health problems such as cancer, heart disease and stroke. According to the American Heart Association’s 2004 Heart Disease and Stroke Statistical Update, approximately 300,000 adults in the U.S. die of obesity-related causes.
